I am IN NETWORK for UHC COMMMUNITY PLANS.

NEW Patients:

All new patients must call the office at 347-669-0017 to book a new patient appointment. If you cannot call during the below times, just leave a message and we will get back to you. 

Hours for telephone service: 

M-F: 9:30am-1:00pm

If you need to send medical records ahead of your visit, please download, sign, and send this release form (Release of information – UFD) to the medical provider who has the records to be released. They will then send them to our office. 

ESTABLISHED Patients – Book through the patient portal or call the office. 

Next day / same day visits available for urgent concerns.

Days of operation are generally:
Mondays, Tuesdays, Thursdays and 2nd Saturdays (subject to variability)

We are CLOSED on Wednesdays and Fridays, but we can be reached via telephone. All voicemail messages will be returned promptly.  

If you can’t find a time that works for you,  just call or text 347-669-0017 or email and ask! We can be flexible for earlier or later appointments on a case-by-case basis.
